Attendees are responsible for their own hotel reservations. Be sure to mention the AMA’s 2018 CPT Symposium when booking. The group rate is only applicable November 14 – 18. Hilton cannot guarantee room availability after the published cut-off date. Rooms are available on a first-come, first-served basis; no additional rooms will be added. The AMA is unable to assist with room reservations during the Symposium.

REGISTRATION AND CANCELLATION POLICIES AMA reserves the right to modify or cancel any event due to unforeseen circumstances. Registered attendees will be notified of cancellation in advance of the event and will receive a full refund of monies paid for the cancelled event within 30 days of notification. The event will not be rescheduled. AMA assumes no responsibility for personal expenses.

REGISTRATION Registration for an event may be completed in one of three ways: (1) online, (2) telephone, or (3) US mail. The AMA does not accept registration by fax or email submission. Onsite registration is available only for the CPT® and RBRVS Annual Symposium.

Payment may be in the form of credit card or check made payable to American Medical Association. Payment by credit card cannot be accepted when registering by US mail. Registration must be paid in full prior to the first day of the event.

CONFIRMATION OF REGISTRATION Confirmation letters will be sent by email delivery only after registration is completed. A valid email address must be provided when registering. If you do not receive a confirmation email, please contact Wendy Carranza.

REFUNDS AMA provides full refunds less a processing fee of $125 for registration cancellations made at least 46 days prior to the first day of the event. A partial refund of 75% is provided for registration cancellations made between 45 and 31 days prior to the first day of the event. NO REFUNDS are provided within 30 days of the first day of the event. All refunds are processed within 30 days of receipt.

SUBSTITUTIONS Substitutions (that is, assigning a paid registration to another individual) are accepted through 15 days prior to the first day of the event. A processing fee of $50 will be invoiced at that time. Original registrants who release their seat for substitution forfeit all CEUs for completed course work. All fees must be paid prior to the program dates. Substitutions are not accepted within 14 days of the program date; no refund will be provided.

NO SHOWS Those individuals who have paid but do not show for an event must request that their event materials be shipped to them. CEU certificates will not be awarded because attendance at the event is required. Materials will ship through regular USPS mail using the address provided at the time of registration.

SUBMITTING REQUESTS FOR REFUNDS, SUBSTITUTIONS AND NO SHOWS Requests must be submitted by email to Wendy Carranza. For refund and no show material requests, please provide your name, mailing address, phone number and email address, along with the name and date of the event. In the case of a substitution, please include the following information for both the original registrant and the substitute registrant: Name, mailing address, phone number, and email address. Notification of approval will be made within seven days of receipt of the request. Requests will not by accepted by telephone or onsite.
